Home From Hospital care that helps you settle back in
Practical, personal support at home after a hospital stay, planned around your discharge needs, routines and recovery.

Planning ahead
Planning for a safe return home
The best time to consider support is before discharge, where possible. We can discuss what help will be needed, what has changed since admission and whether the home environment needs practical preparation.
Where our role begins and ends
Clear boundaries, for your safety
Home From Hospital care is practical, personal support, not a substitute for clinical decision-making. These three principles hold regardless of how urgent or complex a return home feels.
1
We do not replace the hospital discharge team or clinical professionals
Our role is to provide the agreed care and practical support once you are home, clinical decisions and discharge planning stay with the hospital and your healthcare professionals.
2
Medication instructions are verified, never guessed
The current medication list and discharge instructions should be clear before carers provide support. If instructions are unclear or don't match current records, they're checked with the appropriate healthcare professional rather than assumed.
3
Two-carer transfers are planned, never improvised
Moving and handling support is based on an up-to-date assessment and the correct equipment. If two carers are required for a transfer or repositioning task, that's planned in advance rather than asking one carer to manage an unsafe situation.
Support after discharge
Care planned around what you need when you get home.
Personal care and mobility
Help with everyday routines and moving around safely.
Meals and medication
Agreed support with food, drinks and prescribed medicines.
Short-term or ongoing
Care can be reviewed as your needs change.
Medication after hospital
Medicines often change during a hospital admission. The current medication list and discharge instructions should be clear before carers provide support.
Care & Carers can support the administration of prescribed medication according to the assessed care plan.

Hospital stays can disrupt appetite, sleep and usual routines. A carer can help with agreed meal preparation, drinks and everyday tasks while you settle back into your own surroundings.
length of support
Temporary support or longer-term care
Some people need extra help for a short period. Others discover that ongoing home care would make day-to-day life safer or easier. We can review the care as you recover or as your needs become clearer, reducing, increasing or changing the support where appropriate.
Short-term
Extra help while you regain confidence and settle back in, reviewed as you recover.
Ongoing
The start of a longer-term care package, if that turns out to be what day-to-day life needs.
