Home Care & Support

Home Care That Helps You Keep Living Life Your Way

Flexible, personalised care in your own home, from a little help with everyday routines to regular support throughout the day.

Home care can make everyday life easier without asking you to give up the home, routines and independence that matter to you.

At Care & Carers, we start by understanding what you can do, what you would like help with and how you prefer that help to be provided. We then build visits around your assessed needs and normal routine.

What Is Home Care?

Home care, sometimes called domiciliary care, is support provided in your own home. A carer visits at agreed times to help with the parts of everyday life where you need assistance.

For one person, that may mean help getting ready in the morning. For another, it may include meals, medication, mobility, personal care and an evening visit. The amount of care should reflect the individual rather than a standard package.

Who Can Home Care Help?

Home care may be useful when you are managing well in many areas but need some support to keep doing so safely and comfortably. It can also provide more substantial daily help where needs have increased.

What Can a Home Carer Help With?

Your care plan sets out exactly what support has been agreed. Depending on your assessed needs, visits can include personal care, medication support, meals and drinks, mobility assistance, companionship, shopping, light household tasks and help getting out and about.

Getting Out & About
The aim is not to take over. It is to provide the right support so you can keep as much choice, control and independence as possible.
how visits work

Your Time, Your Way

Care should fit around everyday life rather than make everyday life fit around care. We want to understand when you prefer to get up, what you like to eat, how you want personal care to be approached, which routines matter, and what you want to do for yourself.

Those details help turn a list of tasks into care that feels personal.

Home Care for Changing Needs

Care needs can change gradually or suddenly. If your circumstances change, for example, after a fall, illness, or hospital stay, the care plan should be reviewed rather than assuming the existing arrangement is still appropriate.

Where visiting care isn't enough

Where visiting care is no longer sufficient, we can discuss whether a different arrangement, such as live-in care or night care, may be more appropriate.

Support for Families

Families often play a major part in helping someone stay at home. Our role is not to replace that relationship. It is to add reliable professional support around the person and the people already important to them.

Information is shared with family or representatives according to the person's wishes, consent and agreed care arrangements.
